Barbituric acid, first synthesized in 1864, is the parent compound of barbiturate sedatives. Its derivatives like phenobarbital and pentobarbital once dominated the sedative-hypnotic market for nearly a century. However, the safety profile is a major concern—barbiturates have a narrow therapeutic window, with respiratory depression risk at slight overdose. Benzodiazepines, introduced later, offer wider safety margins and have largely replaced barbiturates in clinical use. Today, barbituric acid's role has shifted from direct drug use to an intermediate for producing other pharmaceuticals, including certain antibiotics and vitamins. This transition reflects a structural decline in legacy sedative applications, while niche pharmaceutical synthesis continues to sustain baseline demand.
